Thank you for choosing Mir Neurology and Spine Center, we’re grateful you’ve decided to trust us with your care. This page will help you plan your visit to our clinics and will also provide downloads of all relevant patient forms.
We look forward to welcoming you to one of our centers. In order to give you the best care possible, Dr Mir and his team will need as much information about you and your condition as possible. To that end, please fully complete all the patient registration forms found on this page and bring them with you to your first appointment.

Appointments
We see all patients on an appointment only basis, and each new patient consultation will last 30 minutes. You can book your appointment by calling us on (301)-797-7600 or via our secure online contact form.
We try and see patients at their scheduled time – and this is the case in the vast majority of visits. However, sometimes the clinic will run behind schedule due to unforeseen patient emergencies – and in such instances we ask for your patience and understanding.

Missed Appointments
If you’re unable to make your appointment as scheduled, please phone us to cancel or reschedule your appointment (301)-797-7600. We request that you provide us with a 48 hour notice prior to your appointment. If you no-show your scheduled appointment, you will be assessed a $35 fee.
