Preparing For Your Visit & Patient Form Downloads

Essential Information

Thank you for choosing Mir Neurology and Spine Center, we’re grateful you’ve decided to trust us with your care. This page will help you plan your visit to our clinics and will also provide downloads of all relevant patient forms.

We look forward to welcoming you to one of our centers. In order to give you the best care possible, Dr Mir and his team will need as much information about you and your condition as possible. To that end, please fully complete all the patient registration forms found on this page and bring them with you to your first appointment.

On the day of your visit, please also bring the following documentation with you:

  • A credit card
  • Your insurance cards
  • A photo ID
  • Your prescription card
  • A valid referral (if required for insurance purposes)
  • Your co-payment for your appointment

It would also help us greatly if you brought with you any of the following medical records – or ask your physician to send them to us directly. These records could include:

  • Consultation reports
  • Pathology tests
  • Lab reports
  • Radiology tests
  • Surgical procedure reports

Appointments

We see all patients on an appointment-only basis, and each new patient consultation will last 30 minutes. You can book your appointment by calling us on (301)-797-7600 or via our secure online contact form.

We try and see patients at their scheduled time – and this is the case in the vast majority of visits. However, sometimes the clinic will run behind schedule due to unforeseen patient emergencies – and in such instances, we ask for your patience and understanding.

Missed Appointments

If you’re unable to make your appointment as scheduled, please phone us to cancel or reschedule your appointment (301)-797-7600. We request that you provide us with a 48-hour notice prior to your appointment. If you no-show your scheduled appointment, you will be assessed a $35 fee.

These payments are part of our office policy for non-cancellation of appointments and are not covered by your insurance.
